Supporting the systemic development of integrated care plans for patients of the Rare Disease Centre (Ośrodek Chorób Rzadkich – OCR) operating within the University Clinical Centre (Uniwersyteckie Centrum Kliniczne – UCK) – developing a system for Duchenne muscular dystrophy (based on international TREAT- NMD guidelines).

Challenges

Development of a set of procedures based on international standards for DMD care (80 paediatric patients) and use and development of the Medical 4.0 system integrating data to implement the developed procedures
Ensure consistency of data acquired into the system from various sources
There were no systems on the market for determining integrated care plans for patients with rare diseases, allowing the collection and analysis of current and historical patient data for determining, but also modifying the integrated care plan for chronically and severely ill patients
Remote monitoring of the patient’s condition

Actions

Generate, optimise and implement procedures in the Medical 4.0 system for neurological, nutritional, rehabilitation and cardiac care, as well as remote patient monitoring
Defining the range of data to infer, finding correlations between them enabling faster inferences about the patient’s condition and further treatment
Adaptation of the system for data entry via mobile devices (e.g. smartphones, tablets)

Effects

Enabling direct transfer of data from the echocardiographic device (ECHO examination) to forms
Automatic data analysis (the system detects exceeding of control values in a given patient’s results and allows both system and email notifications to be sent to caregivers or physicians)
Establish a so-called cardiac alert and develop warning and response procedures
Remote monitoring of the patient‘s condition (e.g., from the patient’s home)
Caregivers via email, get information about the date of the consultation, about the dates of taking measurements at home, up-to-date information about the treatment plan, recommendations for changes, information about the date of the next or additional consultations, if the system together with the team of specialists draws attention to worrying changes
Storing data in the right quality (catalogued, correct, up-to-date, full-value data)
Historical data import
Reporting/exporting data for reuse in other studies/analysis/in AI and ML
